Your Care Team

Our care team consists of

• Physicians - Manage comprehensive rehabilitation and medical care.
• Physician Assistants (PA) - Assist the physician in managing comprehensive rehabilitation care.
• Rehabilitation Nurses - Assumes responsibility for management and deliver of patient care.
• Physical Therapists (PT) - Provides interventions to improve the ability to perform transfers and mobility.
• Occupational Therapists (OT) - Provides interventions to improve the ability to perform daily activities.
• Speech Language Pathologists (SLP) - Provides interventions to improve abilities for swallowing, memory, and voice.
• Recreational Therapists (RT) -  Provides rehabilitation through recreational and leisure activities.
• Music Therapists (MT) -  Provides rehabilitation through music.
• Counselors - Provides individual counseling (and family counseling), implementing interventions based on patient specific needs.
• Case Managers and Social Workers - Assist patients with coordination of services and discharge planning.
• Rehab Admission Liaison - Screens patients for eligibility for inpatient rehabilitation services.
• Rehabilitation and Nurse Technicians - Assists clinicians in various facets of patient care.
