Mission
Help transform Medicare, Medicaid, and the Children's Health Insurance Program (CHIP) through improvements in the health care system, thereby ensuring better health care, better heath, and reduced costs for beneficiaries, and ultimately enhancing the health care system for all Americans.
PCMH Project and Participating Practices
INTEGRIS Health along with 15 other Organizations around the United States participated in a three year demonstration project. This is just one of several innovation projects sponsored by the Centers of Medicare and Medicaid Services (CMS) which focuses on improving the health of our American population, ensuring better health care as well as reducing healthcare costs.
Project Goals
Goal One: Decrease overall health care cost for Medicare and Medicaid beneficiaries by reducing the total cost of care.
Goal Two: Improve the health of the patient population by demonstrating improvement in each of the selected quality measures.
Goal Three: An improvement in the patient experience which is reflective in patient engagement, access to care/information, and quality.
Goal Four: Demonstrate scalability by the spread of Medical Home and Medical Neighborhood learning to additional practices within the community.
Outcomes
June 30, 2015 marked end of the 3 year CMMI PCMH Demonstration Project.
