Our programs offer aftercare management and monitoring for one year after discharge, at no additional cost. During this time, we help coordinate relapse prevention groups and other support depending on your local community. You’ve been through the inpatient process, but the days you spend in a recovery center are just the start of the journey. Adjusting to life after recovery can be challenging when faced with the external factors and influences that may have impacted your addiction. At Arcadia Trails, we help you continue your health journey through sober-living arrangements, ongoing group and individual therapy, support groups and year-long monitoring and management.
Aftercare programs are designed prior to discharge with the specific patient and their needs in mind. We will follow up post-discharge at one week, one month, six months, and one year to help you continue your success.
Our programs continue the work and support you found during the residential recovery program to help you stay in recovery long after leaving Arcadia Trails. We also have a large network of community resources that can help with challenges you may face during this time.

Each patient at Arcadia Trails receives a personalized treatment plan, and this is exactly how our aftercare plans are formatted. It is so important to meet patients’ needs individually to prevent re-occurrence.
Each patient creates a re occurrence response and re occurrence prevention plan before they discharge. Patients are made distinctly aware of the challenges they will face in their newfound sobriety.
Many organizations, programs and resources exist to help those living in recovery thrive, in addition to the steps that recovering patients can take themselves to find support. We also stress the importance of sober living arrangements, outpatient treatment and therapy, as well as, connecting with peer support and attending support groups.
Having support is one of the top ways aftercare helps during the recovery process. Support groups are vital in remaining sober because they offer a judgment-free atmosphere where members talk about their issues, find support from like-minded friends and find advice and information from a supportive group of people.
Therapy groups and services are tailored to each patient and their vision for recovery. Everyone is offered a unique plan in their specific community with recommendations that would offer them the most likely situation to succeed in long term recovery.
Support groups are also selected with the patient in mind. If you enjoy the traditional 12 Step approach or a more specific spiritual or non-spiritual approach, we base your plan on your individualized needs and community offerings.
Arcadia Trails has case managers who make it their goal to be familiar with the different resources in each patient’s home town. We also have a close relationship with the pre-existing sober living communities. We provide access to those representatives through meet-and-greets with patients here at Arcadia Trails where they can complete applications and get to know the personalities of each organization.
