Anticoagulation Clinic 

We provide management and monitoring of anticoagulation therapy with a primary focus on warfarin (Coumadin®) therapy. The clinics provide focused patient assessment and education visits targeting areas of adherence, drug interactions, dietary considerations, bleeding and thrombosis.

Laboratory (INR) testing is done by finger stick with the CoaguChek XS device. This allows for quick feedback on the patient's response to warfarin and immediate instructions for warfarin adjustments.

The clinic works in collaboration with the referring physician to provide optimal patient outcomes, while minimizing physician time spent on providing routine monitoring.


Additional information

The INTEGRIS Health Anticoagulation Clinics are outpatient hospital-based clinics, and therefore, accepts all insurances accepted by the INTEGRIS Health hospital with which the clinic is affiliated. Billing statements are sent out monthly by the INTEGRIS Health billing office. Co-payments are due at time of service.

The initial clinic visit includes a comprehensive medical and medication review. In collaboration with your physician, we will establish your therapy goals and talk about what to expect during your anticoagulation therapy. Education materials will be reviewed, and your questions will be answered. A blood test to measure your clotting time (INR) will be tested by a finger stick. Any adjustments in your therapy will be determined and communicated during your visit. The initial visit is approximately 45-60 minutes in duration.

At each follow-up clinic visit, we will reassess your anticoagulation therapy including an evaluation of drug and diet interactions, health changes, and adverse effects related to therapy. A blood test to measure your clotting time (INR) will be tested with a finger stick. Any adjustments in your therapy will be determined and communicated during your visit. Follow-up visits are approximately 10-15 minutes in length.

The clinic operates according to medical-director approved protocols and guidelines and in collaboration with the patient's personal physician (referring physician). The referring physician is provided routine communication on patient progress throughout their care in the clinic.

Criteria for patient enrollment in the Anticoagulation Clinic

  • Referring physician is providing ongoing medical care to the patient and maintains the collaborative practice relationship with the clinic.
  • Patient is ambulatory and able to come to the clinic for appointments. The clinic may provide short-term phone management for patients upon the discretion of the clinic. The clinic will not provide long-term phone management for patient on home health, hospice, or those getting venous punctures at other sites with INR results faxed to the clinic.
