Now that you have identified your health care wishes and values, appointed your health care proxy, and completed the appropriate documents you can a few more steps to take in your advance care planning process.

  1. Provide copies to family members and especially to your health care proxy.
  2. Take a copy to your primary health care provider
    1. Review your advance directive with your provider
    2. Have your provider scan the advance directive into your medical record.
  3. Keep your advance care planning documents in an easily accessible location. Bring a copy if you are admitted to the hospital.
