The INTEGRIS Continuing Care Network (ICCN) is a clinically integrated network of INTEGRIS Health post-acute care providers and NON-INTEGRIS preferred post-acute care providers.
The INTEGRIS Continuing Care Network (ICCN) is a clinically integrated network of INTEGRIS Health post-acute care providers and NON-INTEGRIS preferred post-acute care providers. The network’s aim is to use care coordination and evidenced based clinical pathways to drive cost efficient and effective quality care of designated targeted patient populations throughout the defined continuum of care.
The ICCN is governed by a Steering Committee with the purpose to provide multi-disciplinary, system-level oversight to help guide the planning, development, promotion, implementation, and strategic development of post-acute care service provision for INTEGRIS Health. The ICCN Steering Committee also seeks to help improve the overall patient care experience, improve the health of the patients and communities we serve, and reduce the cost of care.
With the oversight of the ICCN Steering Committee, preferred provider networks have been developed between Skilled Nursing Facilities/Home Care agencies and INTEGRIS Health. These networks meet on a regular basis to improve patient outcomes, reduce care variation, and strengthen care coordination by establishing and monitoring quality and performance metrics within the INTEGRIS Health preferred network.
