The INTEGRIS Continuing Care Network (ICCN) is a clinically integrated network of INTEGRIS Health post-acute care providers and NON-INTEGRIS preferred post-acute care providers.
At INTEGRIS Health, we understand finding the right place for continued care is hard for patients and families and wish to support your choice. We have developed a preferred provider network with post-acute facilities. The goal of our network is to improve care continuity, quality and efficiency for our patients needing additional care. We chose to work with these providers based on previous patient preferences, satisfaction, and quality outcomes.
