INTEGRIS Health will host an Advance Care Planning Drive to celebrate Healthcare Decisions Day on April 16, 2022.
We encourage INTEGRIS Health patients to bring their Advance Directives, Medical Power of Attorney and Do-Not-Resuscitate documents to be uploaded to their INTEGRIS Health chart. We also want to invite members of the public to the Advance Care Planning Drive to complete their own documents if they haven’t already.
We will have a team of
Advance Care Planning coordinators to assist anyone with any questions and completing the Advance Directive and Do-Not-Resuscitate forms. If you have any questions or would like to send your ACP documents electronically, you can email us at
