Annual Caregiver Wellness Program Requirements 2026

1. Complete the Health Risk Assessment survey in your ReadySet Portal
2. Complete a Wellness Visit with your Provider
3. Complete the biometric bloodwork panel ordered by your Provider (CMP, Lipid Panel, Hemoglobin A1C)
4. Upload the Wellness Verification Form or the Maternal Wellness Form completed by your PCP OR a verifiable document indicating preventive services were completed to ReadySet
