  Offering a secure suite of online tools including the ability to schedule appointments, access medical records, send messages to your provider’s office and more. To ensure the most efficient experience, consider activating your INTEGRIS Health & Me account before your appointment. Parents and legal guardians of pediatric patients may request proxy access to access these online tools on behalf of their child. To request proxy access, please complete the minor proxy request form and submit it electronically via email or fax prior to your child’s appointment or bring it to the appointment.
