Rehabilitation in the Home Care Environment

Course Description:

This course is designed to teach participants about rehabilitation in the home care environment according to regulatory standards. Participants will develop an understanding of admission criteria as defined by CMS, discuss various patient scenarios in regards to how to determine the best level of care required by individual patients/families, preparing patients/families for a home care admission, and the development of patient centered goals when providing care in the home care environment.

Course Outcomes:

At the conclusion of this course, participants will be able to:

1.Facilitate smooth transitions across the continuum of care for our patient population
2.Describe criteria for admission to home care as defined by CMS
3.Explain services and disciplines available in the home care setting

Speaker Bio:

Shylia Eales:  Graduated from OU in 2004 with Bachelor’s in Health and Exercise Science and from OUHSC in 2008 with Master’s in Physical Therapy. Practiced in outpatient setting for 2 years and have been in home care for the past 9 years.
Derrien Relyea:  Graduated from OCCC in ’89 with degrees in both OT and RT, and from OUHSC in ‘98 with a degree in PT. Derrien maintained dual licensure for many years, in both OK and TX. She was rehab director for a major corporation for 4 years doing pediatric outpatient then was in charge of therapy at a small hospital for over 7 years and taught new employee courses in peds and wound care. She has experience in sensory integration, NDT, ICU, ER, inpatient, outpatient, HH, Hand therapy, ortho, neuro, peds, adult and geriatric rehab.
Jamie Hall:  Jamie graduated from OCCC in 2005 with her ADN. She worked for a couple of years at OUMC in both adult and pediatric med/surg before joining Integris Home Care in 2008. Jamie worked in home care as an RN case manager for several years and became the Clinical Educator in 2017. Jamie returned to school and received her BSN in 2018 and transitioned to the role of Home Care and Hospice Clinical Education Consultant in 2018.
Tracy Turner: Tracy graduated from Texas Women’s University and has 28 years’ experience working as an OT. She has spent 27yrs working in home care and 18 of those years has been spent here at Integris Home Care.
