INTEGRIS Health Partners Network is an innovative service that allows you to team with your physician to improve your overall health. Collaborating with your health in mind encourages excellent primary care, preventive screenings and better care for chronic conditions. At INTEGRIS, our approach is simple – to improve the health of the people and communities we serve.

Choosing a primary care physician to lead your care team is optional, but strongly encouraged. It's easy to find one if you visit

  1. Type a name, specialty, procedure or condition in the "Who or what are you looking for?'' box, or select "Doctors (Primary Care)" from the list provided.
  2. Enter your ZIP code in the "Where?" box.
  3. Choose (OK) Aetna Whole Health-INTEGRIS Health Partners Network from the "Select a Plan" drop down menu.

For patients with chronic conditions such as hypertension, hyperlipidemia, diabetes, asthma and cardiovascular disease, our staff will coordinate with you to manage your health care. Our hope it to help you achieve fewer sick days and improved overall wellness. As part of your care team, our nursing staff is dedicated to assist you in your journey to better health. They are available to answer questions about the INTEGRIS Health Partners Network, connect you to appropriate services an d providers and supply health coaching and resources to help you achieve improved health.
